Care Management
Enrollment, care plans, documented time and program visibility for the care that happens between visits — running on the same chart as the clinic, so a coordinator and a clinician are looking at one patient rather than two records.
From enrollment to billing readiness
Most teams start this work in a spreadsheet and end up maintaining two versions of the truth. Here every step is recorded against the patient as it happens, so the reporting is a by-product of the work rather than a monthly reconstruction.
What it handles
-
Enrollment
Add patients individually or import in bulk. Program assignment and consent are structured fields rather than notes, so they can be reported on and reviewed later.
-
Care plans
Goals and interventions attached to the program the patient is enrolled in, updated as the plan changes instead of buried in free text.
-
Time and visits
Encounters and minutes captured as part of the coordinator’s work, with the detail supervision and billing preparation require — not reconstructed at month end.
-
Daily worklist
Filters, priorities and follow-ups that decide who to contact today: who is overdue, who is high-risk, who is due a check-in.
-
Dashboards
Role-aware views — enrollment and activity for leadership, workload for managers, today’s list for coordinators — all reading the same data.
-
Program configuration
Structure chronic care, behavioral health, transitional care and other services to match how your contracts are actually written.
Who uses it
One product, three honest views of the same work.
-
Care coordinators
A daily list that says who to call, what is overdue and where the last follow-up went — replacing the spreadsheet most teams start with.
-
Program directors
Chronic care, behavioral health and transitional care in one view rather than three separate trackers that disagree.
-
Quality and operations
Enrollment, activity and billing readiness in a form that survives review, with the history behind each record intact.
A coordinator’s morning
- Open the worklist. Overdue, high-risk and due-for-outreach patients, sorted for the day.
- Open the patient. Last visit, active program, current goals and the plan on one screen.
- Update the plan. Mark goals complete, add interventions, schedule the next follow-up.
- Document the time. Minutes and detail recorded against the program, ready for billing preparation.
How it fits the rest of iCare
Care Management is a module of the iCare Enterprise Cloud EHR, not a separate application. Enrolled patients are the same patients in the clinical record, so a coordinator’s outreach and a clinician’s note sit in one history.
Access is role-based and record history is retained, so activity can be reviewed after the fact. Protected health information is handled under the HIPAA Privacy and Security provisions and business associate terms of the iCare Online Client Agreement.
